Patient Registration

To register as a new patient online please complete the form below making sure you FULLY COMPLETE both the NHS Family Doctor Services Registration Form and the New Patient Questionnaire that follows it. If you do not complete both of these forms fully we will be unable to register you with the practice. You must provide the surgery with two forms of I.D at your first visit.

It is very important that all new patients wanting to register with the practice book an appointment for a health screening examination with our healthcare assistant. This normally takes a maximum of 20 minutes. Patients are registered with the practice and with a GP for administrative purposes only.

Named accountable GP

NHS regulations state that all patients are to have a named GP responsible for their care.

All our patients, including children have been allocated a named, accountable GP.

There is no requirement to write to any patients regarding their named GP. Practices are required to inform patients of their named GP at the next appropriate interaction.

You may request a specific named GP (requests will be considered) and can contact the practice to find out who your named allocated GP is.

What does 'accountable' mean?

The contract requires the named accountable GP to take responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).

You do not have to see your named GP and can request to see any available GP the same as you can at present.

9 Confirm

Although sending information across the Internet means that privacy of data cannot be absolutely guaranteed, our online forms offer high levels of security. The forms use HTTPS and TLS (using a 2048 bit certificate) to encrypt data sent to and from your computer when using the site and our server environment is PCI DSS certified - the highest standard of security associated with e-commerce and online finance solutions.

Form submissions are stored on the server for 28 days and are encrypted and secured with a private key known only by the GP Practice.

Should you have any concerns about sending your personal details using the web, please use one of the alternative methods offered by our organisation.
