Warwickshire North's CCG Patient & Public Engagement Champions project, enables local residents to get more involved with the development of their health care services.
If you are interested in becoming a Health Champion, please fill in the form below and help your NHS get better.
Your information will be held by NHS Arden & GEM Commissioning Support and you will receive regular updates and surveys from local NHS Clinical Commissioning Groups in Coventry and Warwickshire. We will not share your data with anyone without your explicit consent. Your data will be kept in accordance with the NHS confidentiality code of practice.
