JUBILEE HEALTH CENTRE – complaints or comments about the Health Centre in general should be made to the Building Manager via the Information Desk. Some services at the Health Centre are provided by hospital or community services and complaints about those services should be made to the relevant provider.
GENERAL COMMENTS & SUGGESTIONS – please use the form below to submit your general comments or suggestions, this may not result in a direct response from the practice. This form is not for complaints, if you have a formal complaint please refer to our complaints procedure. If you wish to ensure a response from the practice please do not use this form.
