Access to Medical Records

From 1st April 2016 this practice offers the facility for patients to view online a summary of their detailed coded Medical Records, including Medication, Allergies, Laboratory Test Results, Problems, Immunisations and Adverse Reactions.

To register you need to:

Present in person at the Health Centre

Provide two forms of documentation as evidence of identity, one of which must contain a photograph. Acceptable documents are Student ID Card, Driving Licence, Passport, Bank Statements or a Utility Bill.

Complete an Application Form. This will be forwarded to a GP for authoristation prior to access being granted. Please allow a minimum of 7 days for this to be actioned.
