To register with the surgery, you and your family will need to live in the practice area. Please click to check our catchment Practice Area
If you live in the catchment area,
- Please fill the online form, you will need to bring your Photo ID and 2 proofs of address [dated within past 6 months] to the surgery or drop it in the letterbox on the gates. OR
- Please download the registration form, you will need to bring your completed registration form, Photo ID and 2 proofs of address [dated within past 6 months] to the surgery or drop it in the letterbox on the gates. OR
- Visit the surgery to get the advice from receptionist for registration.
Children – Need Photo ID please.
Once the application along with ID and address proof has been verified, you / your family will be contacted through a letter or by telephone call. All new applicants including children will need a new patient registration check with practice nurse. If child(ren) aged under 5years please bring the Redbook / child Health Records to the appointment.
We shall endeavour to process your application as quickly as we can usually within 2 to 3 weeks provided all support documents and the forms filled completely.
In meantime if you need to see a GP / Nurse URGENTLY for your medical need, we will try to accommodate you or advise you to the nearest walk-in centre.
We do offer Patient Online Services to book and cancel your appointments with Doctors only and Order your Acute and Repeat medications except some medications. Now you could also view parts of your GP health record, including information about medication, allergies, vaccinations and previous illnesses
